The same failures come back, and every one of them was closed properly.

Take the stoppages that cost you most this year and count how many were the second or third time that machine stopped for that reason. Then look at how those jobs were closed: completed, on time, by someone competent, signed off. Nothing in the closing of a job asks whether the cause was removed, so a repeat and a first occurrence are written the same way, and cost you the same way twice.

Three things change for the person who owns the number.

Commit an availability figure you can defend.
Not because the number is promised, but because what sits under it is visible: which machines are repeat offenders, which are unmeasured, and which stoppages were booked to categories that do not count.

Move spend from the shelf to the check.
A critical spare held against a machine nobody can name is working capital. A standing check on the three machines that keep coming back is a line item you can justify, and it is smaller.

Stop funding the war room.
The people who assemble the answer after the stop are your most expensive people. When the relation is held before the stop, that meeting becomes a ten-minute read, and Thursday goes back to the problem he was hired for.
Before we start, three things have to be true

A maintenance record that says what was found.
Not what was done. What was there when the machine was opened. Everything above rests on this one thing.

Failures named consistently enough that a repeat is recognisable.
If the same fault is written three different ways, it is three faults, and the pattern never forms.

Somebody who can change a plan.
A proposed check that nobody is empowered to accept is a finding that dies in a meeting.
And if your failures genuinely arrive with no prior signal at all, no repeat, no history, no drift, nothing a good engineer would flag in hindsight, then this is bad luck rather than a hidden warning, this page is not for you, and we will say so.
